Service User Care Plans
Care Plans for Service Users are tailored individually based on assessed needs, wishes and preferences with reviews conducted at least every six months. We understand the importance of personal fulfilment for every individual. Therefore, we create tailored care plans with meaningful activities that address the unique needs of each service user, helping them achieve their personal goals, maintain their independence, and live safe, happy, and fulfilling lives.
Therapeutic Activities
A strong emphasis is placed on maintaining Service Users’ social networks and activities. Each Care Plan includes space to document the Service User’s life history, social connections, and preferences for hobbies and activities. This ensures access to social opportunities and activities that are both appropriate and aligned with the Service User’s interests and desires.
